If you are a parent of a school-age student in grades 1-12 who is a registered BBFN member, please complete the form and send it back to me, or pick one up at the front desk.

** Please make sure you have your student’s status card numbers in full, or it will be delayed **

All forms received prior to Sept 8th will be processed by September 10th. Anything received Sept 9th onward will be processed the following week.
